Job Description:

The Physical Therapist will provide professional and comprehensive physical therapy treatment to individuals in the home setting under the direction of the Director of Nursing. He/she will assess patients' needs and formulate, implement, evaluate and develop the plan of care. Will also perform primary case management function of assigned patients. This professional will work closely with physicians and will provide updates on changes in patient’s condition and care needs.

Other Responsibilities:

• Assess, monitor, and intervene as appropriate with respect to patient's physical and emotional needs. Develop appropriate goals with patient and family.
• Develop and coordinate accurate written plans of treatment with physicians and others involved in the care of patients, utilizing appropriate care plans and protocols.
• Update care plan to reflect change in physician orders or patient condition and evaluate at least every 60 days and at discharge.
• Maintain documentation in patient electronic record per internal, professional and regulatory standards. Complete additional updates as required.
• Supervise care provided by PT Assistants every. Modify HHA care plan with respect to the exercise program and provide written instruction to the HHA of the same.

Education Required:

• Current Physical Therapist license in Illinois
